Lay your shirt flat without stretching it. Measure the chest from underarm to underarm and the length from the highest shoulder point beside the collar to the hem. Width is one side of the shirt, not chest circumference.

The supplier allows measurement variation of up to 1.5 in (3.81 cm). Measurements describe the garment, not your body.
